Job description
Optum HouseCalls seeks a Nurse Practitioner or Physician Assistant to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This non-prescribing, field-based role focuses on preventive care, patient education, and identifying care gaps in coordination with primary care providers.
Travel statewide across Oregon (≈85%), with all travel expenses covered. Flexible hours (8:00 AM–8:00 PM), no on-call, no weekends or holidays.
