Job description
UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members across Rhode Island and Connecticut. You will conduct comprehensive visits, document diagnoses, educate patients on disease management, and collaborate with PCPs to close care gaps.
Requires active certification, state licensure, and travel up to 85%.
