Job description
Well Care Health is seeking a Licensed Practical Nurse to deliver in-home care, administer medications, and support individualized care plans. This role involves direct patient care in patients’ homes across age groups, with supervision by an RN.
The ideal candidate holds an active Practical Nursing license, CPR certification, and at least 1 year of nursing experience, preferably in home health. Strong communication and collaboration skills are essential.
