Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with on-call coverage and weekends as neededn
Shift Duration: 8 hoursn
Hours Per Week: 40n
Weekly Estimated Pay: $1,830 - $1,930n
Setting: Home health and hospicen
Productivity Goal: 30 points per weekn
Mileage Reimbursement: Provided for eligible traveln
Job Requirements
n Active West Virginia RN licensen
Minimum of 1 year of nursing experience requiredn
Home health experience strongly preferredn
Experience with HCHB and OASIS documentation preferredn
Strong assessment, documentation, and care coordination skillsn
Ability to work independently and manage a varied patient caseloadn
Comfortable with on-call responsibilities and weekend coveragen
Responsibilities
n Assess, plan, implement, and evaluate nursing care for patients in the communityn
Support health maintenance, disease prevention, and case management needsn
Collaborate with interdisciplinary team members to coordinate patient caren
Document patient status, progress notes, and required nursing records accuratelyn
Monitor medications and communicate changes in patient condition to the appropriate providern
Coordinate patient appointments and follow-up care to support timely interventionsn
Provide care plan oversight and assist with staff supervision as neededn
Ensure safe, compliant, and patient-centered care in all assignmentsn
