KTI Health is seeking a travel nurse RN Care Manager for a travel nursing job in Apple Valley, California.
Job Description & Requirements
Specialty: Care Manager
Discipline: RN
Duration: 26 weeks
40 hours per week
Shift: 8 hours, days
Employment Type: Travel
Requirements
2+ years of Acute Care Case Management/Discharge Planning experience
Hospital experience required
Seasoned travel nurse required – first-time travelers not accepted
California RN license required by start date
Strong experience with Care Coordination, Discharge Planning, and Utilization Review
Experience working with HMOs/IPAs
Ability to independently coordinate DME, SNF placement, and ambulance transportation
