Acute Rehabilitation Case Manager – Simplified Job Details
Facility: Virginia Mason Franciscan Health Rehabilitation Hospital
Location: Washington
Position: Acute Rehabilitation Case Manager
Start: ASAP
Shift: 8:00 AM – 4:30 PM
Schedule: Monday–Friday
Guaranteed Hours: 40/week
Weekend: None
Call: None
Openings: 1
Reason: Contractor replacement
Requirements
Case Management experience — required
2–3+ years preferred , but flexible based on experience
Must have a current professional healthcare license as one of the following: RN
Social Worker
Respiratory Therapist
Physical Therapist
Occupational Therapist
Speech-Language Pathologist
Must have appropriate Case Management experience
Acute hospital experience with adult patients is preferred
Strong verbal and written communication
Basic Excel, Word, and Outlook skills
Good organization, time management, and analytical skills
Ability to work independently and as part of a multidisciplinary team
Must maintain patient confidentiality
Certifications
Case Management certification is preferred, not strictly required. Examples include:
CCM – Commission for Case Manager Certification
ARN – Rehabilitation Nursing certification
ACM – Association of Rehabilitation Management
ANCC RN-BC in Case Management
Facility & Unit
60-bed freestanding acute rehabilitation hospital
Patient ratio: Approximately 1:15
Patient population: Adults, including patients recovering from stroke, medical debilitation, and post-surgical conditions
EHR: UDS PRO, MediTech, and Pinpoint
Average rehabilitation length of stay: approximately 12 days
Key Responsibilities / Experience
The candidate should be comfortable with:
Case management in an acute rehabilitation/IRF setting
Utilization Review
Supporting interdisciplinary team/family conferences
Working with nursing, therapy, physicians, patients, and families
Discharge/LOS planning and forward planning
Balancing patient advocacy with appropriate use of healthcare resources
Communicating professionally during difficult insurance or healthcare discussions
IRF team-conference experience is preferred, but the facility will provide assistance/training for this requirement.
