This assignment is ideal for an RN experienced in discharge planning, resource coordination, patient advocacy, and guiding patients through post‑acute care transitions.

Key Responsibilities of the Case Management RN
Conduct detailed discharge and resource assessments
Develop individualized care plans in collaboration with providers and therapy
Facilitate transitions to SNF, rehab, home health, and outpatient services
Perform utilization reviews and insurance coordination
Educate patients and families on next steps, instructions, and community resources
Maintain accurate EMR documentation
Serve as a patient advocate ensuring safe and efficient transitions
Requirements of the Case Management RN
Active North Carolina RN license or compact eligibility
Current BLS certification
Minimum 1–2 years of recent Case Management experience
Strong communication, assessment, and critical‑thinking skills
