Job Summary for RN Case Manager - Acute Care (Medical Patients):
- Manage acute care caseload of medical patients as an RN Case Manager
- Coordinate patient care from admission through discharge
- Assess, plan, implement, and evaluate individualized patient care plans
- Collaborate with interdisciplinary healthcare team members
- Monitor patient progress and facilitate appropriate care transitions
- Advocate for patient needs to ensure quality, cost-effective care
- Communicate with patients, families, and insurance providers regarding care plans and resources
- Ensure compliance with regulatory and hospital policies
- Maintain accurate and timely documentation
- Minimum 2 years of RN Case Management experience required
Med Ninjas Job ID . Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
