Hiring Profile:
Must Have Skills/Experience: discharge planning and care coordination, case management minimum 5 years
Preferred Skillset/Experience: More than 5+ years nursing practice in hospital
Type of Patients on Unit: High acuity, complex, high percentage behavioral health
Personality Fit: flexible, easy going problem solver with calm demeanor and can do attitude
