Weekly Schedule: Full-time, approximately 40 hours per weekn

Travel Radius: Up to 75 miles from the assigned care centern
Patient Load: Approximately 5-8 patients per dayn
Productivity Expectation: 30 points per weekn
Call: Potential rotating on-call coveragen
Schedule Notes: No guaranteed hours; overtime must be pre-approvedn
Charting System: HomeCare HomeBase (HCHB) and PointCare; OASIS proficiency requiredn
Other Details: Local candidates accepted; mileage reimbursement availablen
Job Requirements
n Current Registered Nurse (RN) license in handn
Minimum 1 year of adult home health experience or 1 year of hospice experience, depending on specialtyn
Recent experience with HomeCare HomeBase required; OASIS proficiency required for RNsn
Current CPR/BLS certificationn
Reliable transportation, valid driver’s license, and auto insurancen
Comfortable traveling daily to patient homes and attending periodic office meetingsn
Willingness to cover a broad territory and manage a dynamic home care schedulen
Ability to complete documentation within 24 hours of each visitn
Responsibilities
n Provide skilled nursing care in patients’ homes according to physician orders and agency policiesn
Use the nursing process to assess, plan, implement, and evaluate patient caren
Coordinate and direct care delivery to support safe, high-quality outcomesn
Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses as neededn
Complete Start of Care, wound care, follow-up visits, and other home health servicesn
Travel between patient visits while maintaining timely charting and visit documentationn
Work collaboratively with the care team to support patient needs and continuity of caren
Maintain professionalism, flexibility, and strong communication throughout the assignmentn
