On-Call / Weekend Requirement: Every 3rd weekend and 1 night of on-call per weekn
Hours per Week: 40n
Weekly Estimated Pay: $2,120 - $2,230n

Job Requirements
n Registered Nurse (RN) license requiredn
Associate degree in nursing or highern
Recent home health experience strongly preferredn
Experience with HCHB is a plusn
Strong communication, time management, and documentation skillsn
Ability to work independently while covering a defined territoryn
Reliable transportation for travel between patient visits; mileage reimbursement is provided between visitsn
Responsibilities
n Serve as an RN case manager for a designated territoryn
Complete skilled routine visits, recertifications, discharges, and start-of-care/resumption-of-care visitsn
Deliver compassionate, 100% patient-focused care in the home environmentn
Coordinate care and collaborate with interdisciplinary team members to support positive patient outcomesn
Manage a patient caseload with productivity expectations while maintaining quality caren
Cover assigned patients and assist teammates as needed on days offn
Follow clinical standards, documentation requirements, and care plans accurately and in a timely mannern
Participate in rotating on-call and weekend coverage as scheduledn
