Overview
nn We are seeking a dedicated Registered Nurse (RN) to provide case management and hospice care for patients in the home setting. This is a great opportunity for an RN who enjoys autonomy, strong clinical practice, and building trusted relationships with patients and families. The assignment is 13 weeks in length with the potential for a steady full-time schedule.n

Registered Nurse Job Requirements
n Active Arizona RN license or compact state RN licensen
Current BLS certificationn
Associate degree in Nursing or highern
At least 1 year of nursing experience preferred, ideally in homecare, hospice, home health, or case managementn
Experience with HCHB and wound care preferredn
Strong communication, organization, and time-management skillsn
Ability to work independently and manage a patient caseload in the home settingn
Responsibilities
n Provide case management care for hospice patients in their homesn
Complete skilled nursing assessments and coordinate individualized care plansn
Support patients and families with compassionate education and guidancen
Collaborate with the interdisciplinary care team to ensure continuity of caren
Document patient visits accurately and maintain timely clinical recordsn
Travel within the assigned coverage area to complete home visitsn
Participate in weekend and on-call rotation as scheduledn
