Position Overview
Seeking a Clinical Pharmacist to provide safe and effective medication management across inpatient and/or outpatient settings. The pharmacist will review medication orders, optimize pharmacotherapy, collaborate with healthcare providers, provide patient education, and support pharmacy operations.
Key Responsibilities
Review and validate medication orders and prescriptions for appropriateness and safety.
Dispense medications, supplies, and equipment accurately and efficiently.
Monitor patient medication and nutritional therapies and intervene when needed.
Collaborate with physicians and interdisciplinary healthcare teams.
Provide medication counseling and education to patients and healthcare staff.
Participate in medication reconciliation and transition-of-care activities as assigned.
Respond to medication-related inquiries from physicians and staff.
Support pharmacy technician, intern, resident, and new pharmacist training.
Maintain accurate clinical documentation and required records.
Follow federal/state regulations and hospital policies regarding medication use.
Participate in emergency responses and clinical rounds when assigned.
Current California Registered Pharmacist (RPh) license – requirednPharm.D. degree – requirednAt least 1 year of relevant experience required for submissionnInpatient: PGY-1 residency or at least 2 years of clinical hospital pharmacist experience.nOutpatient: PGY-1 residency or at least 2 years of clinical pharmacist experience.nCandidates may be required to meet additional experience requirements depending on the specialty assignment.nCA license must be in hand.nCandidates must live more than 150 miles from the facility; locals are not accepted.
