Job Summary
nPhysical Therapist (Home Health) in the area is responsible for providing patient-centered therapy services within a designated territory. The role involves delivering high-quality care to geriatric patients through home visits, ensuring timely documentation, and adhering to clinical protocols to optimize patient outcomes.
nJob Details
nnCertifications Required: BLS (Basic Life Support)
nEducation Requirements: Active license in hand
nLocation Requirements: Must have an active drivers license and personal transportation; reliability for flexible territory coverage preferred
nYears of Experience Required: Minimum of 1 year relevant clinical experience in home health or similar setting, including OASIS and proficiency in HCHB (Homecare Homebase)
nSchedule: Days, 8-hour shifts, Minimum of 40 hours per week
nAdditional Job Information:nnTravel within up to a 60-mile radius from the hub/office, occasionally more based on patient needs
nAll travelers are required to have an active drivers license, and personal transportation
nRTO (Requested Time Off) must be submitted at the time of application
nOn-call rotation may be requireddiscuss during interview
nTimely communication and documentation are mandatory
nAll travelers are required to attend a Monday orientation
nModules are billable and completed during orientation
nFormer permanent employees must be separated for a minimum of 6 months before returning as travelers and require HR approval
nTravel Pairs accepted with individual territories and transportation required
