Job description
Role
Travel Home Health Physical Therapy Assistant (PTA) responsible for providing patient care in patients' homes, assisting with assessments, implementing therapy plans, and documenting progress using EMR systems.
Responsibilities
Conduct patient assessments, deliver individualized therapy, document care and progress, and ensure effective communication with patients and healthcare teams. Handle approximately 5 to 6 visits daily while maintaining organizational and documentation standards.
Qualifications & Skills
Active Oregon license, home health experience, strong communication, and organizational skills. Must hold a PTA (Compact accepted) certification.
Key Duties
Implement therapy plans, assist with patient assessments, provide one-on-one care in home settings, and document treatment progress accurately.
