Responsibilities

Deliver PT evaluation, treatment planning, and ongoing re-evaluation for patients receiving in-home physical therapy.

Implement evidence-based interventions (therapeutic exercise, gait and balance training, functional mobility training, manual therapy as appropriate).

Develop and adjust individualized home exercise programs; provide patient and caregiver instruction for safe, independent exercise and activity modification.

Document assessments, treatment plans, progress notes, and outcomes in the patient chart; communicate with the interdisciplinary care team to coordinate care.

Monitor safety, infection control, and device/assistive technology use in the patient’s home; ensure compliance with practice standards and regulatory requirements.

Manage caseload within the assigned geographic area; schedule visits to meet patient goals while maintaining quality of care.

Requirements

Active physical therapy license in the applicable state (or eligibility to practice in that state).

Relevant experience preferred, with ability to work in a home-based care setting.

Must have reliable transportation to patient homes within the assigned ZIP-code areas.

Strong interpersonal and communication skills; ability to educate patients and family members on home programs and safety.
