Overview
Physical Therapist, General, Tillamook, OR. Start date: 11/16/2026. Travel assignment for 24 weeks. Weekly pay: $2,576–$2,736 per week. Guaranteed hours: 40 per week. Shift: 4x10 days, day shift (approximately 7:30 AM–5:30 PM). This travel opportunity is in a rural, critical access care setting.

Responsibilities
Conduct initial evaluations and ongoing reassessments to inform individualized treatment plans.
Deliver physical therapy interventions using evidence-based techniques and modalities.
Collaborate with an interdisciplinary team to coordinate care and set patient-centered goals.
Maintain accurate, timely clinical documentation including evaluations, progress notes, treatment plans, and discharge summaries.
Supervise assigned assistants and delegate clinical tasks as appropriate.
Provide patient and family education on treatment plans, safety, and discharge planning.
Requirements
Doctor of Physical Therapy (DPT) degree required; licensure in the state of practice required; license eligibility rules apply if obtained after January 2016 or with foreign equivalent.
Physical Therapy licensure in the state of practice (Oregon) required.
CPR or Basic Life Support (BLS) certification from an approved vendor required.
Two years of Physical Therapy experience preferred.
Ability to work in a rural care setting and meet the 4x10 schedule requirements.
