Job Summary
Physical Therapist (PT) specializing in Home Health (HH) services in a home-based setting. This role involves providing therapy to a diverse patient population across various age groups, managing multiple comorbidities, and ensuring timely documentation of patient visits. The position offers a Monday through Friday schedule with daytime hours.
Job Details: Schedule
Monday through Friday, 8:00 AM to 5:00 PM, with a minimum of 40 hours per week.
Required Qualifications
Valid PT license
BLS (Basic Life Support) certification
At least 2 years of relevant experience
Valid drivers license and car insurance
Willingness to complete all required modules prior to arrival, including MyJourney modules
Preferred Qualifications
Prior home health experience is highly recommended unless otherwise specified
Responsibilities
Conduct physical therapy sessions for all ages
Manage patients with multiple comorbidities
Complete documentation within one day of each patient visit using HomeCare HomeBase electronic medical record system
Additional Information
Regular PT work with flexibility for self-scheduling; manage patient care responsibilities in a home health environment, with all required certifications maintained throughout the assignment; manager or supervisor available 24/7 for support; shifts of 8 hours, 5 days per week, from 10/12/2026 to 01/11/2027.
