Job Details:
Location: Springfield, IL
Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
Required Qualifications: Valid Physical Therapist (PT) license, applicable state licensure, graduate degree in Physical Therapy, minimum of 2 years of experience in home health, and current CPR certification
Preferred Qualifications: Experience with electronic medical records, familiarity with Medicare/Medicaid regulations, and additional advanced certifications in rehabilitation or therapy
Float: Float rotation may be required
Additional Information: The position involves conducting patient assessments, developing treatment plans, and coordinating care with other healthcare professionals. Candidates must be committed to patient-centered care and adhere to all clinical guidelines and protocols.
