Job Details

Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Current license in hand; recent adult home health experience; CPR/BLS certification; reliable transportation and agency-required liability insurance
Responsibilities
Provide physical therapy services to patients in their homes and care facilities.
Conduct start of care visits, follow-ups, and other home health activities.
Document patient assessments and treatment plans using electronic medical records such as HomeCare, HomeBase, Oasis, and Outlook.
Travel up to 75 miles from the home clinic to serve Vance and Warren Counties.
Rotate working a Saturday per month for social on-call needs.
