Job Summary
Physical Therapist Home Health Seaside, OR and Surrounding Areas. This position involves providing outpatient therapy services within a home health setting. The role requires delivering goal-oriented care to enhance patient recovery and independence in their own homes.
Job Details
Certifications: BCLS.
The candidate must have at least 1 year of skilled home health experience, with acceptable certification in BCLS.
Schedule: 8-hour days, minimum of 40 hours per week, starting on 10/19/2026 and ending on 01/18/2027.
The candidate should have experience managing acute care, outpatient clinics, rehabilitation hospitals, and skilled nursing facilities.
Responsibilities include patient assessments, developing treatment plans, and providing therapy services using electronic charting systems such as EPIC and Rover.
The role necessitates own transportation; occasional drive radius may be roughly 60 miles daily.
No on-call duties are required.
First-time travelers are accepted; includes a requirement for references covering 1 year within the past 3 years.
Candidates should be prepared to work in a flexible, team-oriented environment with a focus on patient education and functional improvement.
