Description
American Traveler is seeking a Home Health Physical Therapist with 1+ years of recent HH experience and OASIS proficiency to serve rural and metro territories.
Home Health setting covering Clinton office territory (Henry, St. Clair, & Benton counties, 2x/week) and Kansas office territory (Eastern Jackson and Lafayette counties, 3x/week)
Territories include a mix of rural counties and Kansas City metro area coverage
Day shift schedule, 9:00 AM - 5:00 PM, 5x8 hours
No weekend scheduling required
13-week contract length
Minimum 1 year of recent Home Health experience required
OASIS experience required for staff completing Start of Care (SOC) documentation
Proficiency in HCHB (Homecare Homebase) charting system required
Active driver's license and personal transportation required
License required if applicable
Strong communication and documentation skills required
Responsibilities include timely completion of documentation and communication with the care team
Completion of Home Health Checklist is mandatory prior to starting
Orientation includes completion of billable training modules
RTO requests must be submitted at the time of application
Travel pairs accepted, though each traveler is assigned to their own territory and must have their own transportation
First-time travelers will not be considered for this position
American Traveler Job ID #P- . Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
