Job Summary
Physical Therapist Home Health (HOME HEALTH PT) Located in the region surrounding Mount Angel, Oregon, this position involves providing skilled physical therapy services to adult and geriatric patients in a home health setting. The role requires adherence to agency policies and delivers patient-centered care in accordance with established treatment plans. The position offers consistent weekday hours and the opportunity to serve a diverse patient population in a community-based environment.
Job Details
Schedule: 8-hour Days shift, minimum of 40 hours per week, start date 10/19/2026, end date 01/18/2027
Required Qualifications: Valid state license (compact license accepted), at least 1 year of skilled home health experience, BCLS certification, no on-call required, must have own transportation, COVID-19 primary series vaccination plus booster, and minimum one year of experience in home health or comparable settings.
Preferred Qualifications: Experience with the EPIC charting system, geriatrics, orthopedics, and home health documentation.
Float: Float rotation may be required.
Additional Information: Candidate must provide two references covering a total of one year within the last three years; no direct contact with facility unless arranged. RTO requests and scheduling negotiations must be communicated upfront during submission; refusal to float may impact assignment status. The position includes documentation on a laptop and a work cell phone, with specific dress code expectations. Occasional drive time within a 60-mile radius is expected to optimize patient visits.
