Description
American Traveler is seeking a Home Health Physical Therapist with 1+ years of home health experience to serve patients throughout the Mokena/Frankfort territory.
Details
Home health setting covering the Mokena/Frankfort area territory
13-week assignment on day shift, 8-hour shifts
Provides age-appropriate care to patients ranging from pediatric to geriatric
Mileage reimbursed at the standard IRS rate
Requirements
Minimum 1 year of Home Health PT experience required
Current BLS certification required
IL state license required if applicable
Valid driver's license and car insurance required
COVID vaccination required, with no declinations approved
OASIS and start of care experience strongly preferred
Additional Information
Responsibilities include evaluating, planning, and implementing individualized physical therapy programs to improve patient mobility and independence
Coordinates care with patients, families, physicians, and other healthcare staff in line with the hospital's core values
Completes timely documentation of all patient care activities in the electronic medical record
Must be able to safely transfer and position patients
Local candidates within the service territory will be considered; those too far from the area may be redirected
No more than 5 days of RTO may be requested during the assignment
