Job Summary
Physical Therapist (PT) specializing in Home Health in a day shift setting. This role involves providing outpatient therapy services to geriatric patients in the designated territory. The position emphasizes timely documentation, flexibility with territory, and adherence to clinical standards to ensure quality patient care.
Job Details
Schedule: Days, 8 hours per shift, minimum of 40 hours per week.
Required Qualifications: Minimum of 1 year of relevant experience; BLS certification; active license in hand; proficiency in Homecare Homebase (HCHB) electronic charting system; OASIS experience required for staff completing Start of Care; no first-time travelers; recent experience in outpatient, rehabilitation hospital, or skilled nursing facility settings; valid drivers license and reliable personal transportation; strong communication and documentation skills.
Preferred Qualifications: None specified.
Float: Float rotation may be required.
Additional Information: The traveler is expected to be flexible with the territory and comply with all documentation and communication standards. Orientation will be held on the upcoming Monday. The position involves working with geriatric patient populations, performing assessments, and managing post-op surgeries, spinal conditions, and neurological disorders. Modules are billable and will be completed during orientation. Travel pairs are accepted with each assigned their own territory and transportation.
