Job description
Role:
Travel Home Health Physical Therapist responsible for delivering outpatient rehabilitation services in patients' homes.
Responsibilities:
Provide personalized physical therapy care, develop and implement care plans, educate patients and families, and collaborate with interdisciplinary teams to promote recovery and improve quality of life.
Qualifications & Skills:
Valid Physical Therapist license for the state, home health experience preferred, ability to work independently, and good communication skills.
Key Duties:
Conduct in-home therapy sessions, assess patient needs, document progress, and ensure safety and compliance with healthcare standards.
