Job description
Role: Travel Home Health Physical Therapist
Responsibilities: Provide skilled physical therapy services in a home health setting, focusing on patient rehabilitation and recovery. Responsibilities include assessing patient needs, developing treatment plans, and implementing therapeutic interventions to improve mobility and function. Ensure documentation complies with regulations and maintain effective communication with patients and healthcare teams.
Qualifications & Skills: Valid Physical Therapist license, experience in home health therapy, ability to work independently, strong communication skills, and references confirming clinical competency.
Key Duties: Deliver therapy sessions, monitor patient progress, adapt treatment plans as needed, and uphold quality standards. Maintain accurate records and collaborate with interdisciplinary teams to optimize patient outcomes.
