Job description
Role: Travel Home Health Physical Therapist, responsible for providing patient-centered physical therapy interventions, evaluations, and diagnostic tests in patients’ homes.
Responsibilities
Perform initial evaluations and develop treatment plans.
Re-evaluate patient progress every thirty days and update care plans accordingly.
Help patients regain independence and ensure safe living conditions at home.
Document treatment activities and patient progress accurately.
Participate in on-call duties as required.
Qualifications & Skills
Valid California license and home health experience.
Experience in physical therapy, rehabilitation, or related fields.
Strong documentation and evaluation skills.
Key Duties: Deliver quality physical therapy services in a home health setting, promote patient independence, and collaborate with healthcare teams to update care plans.
