Job description
Role:
Home Health Physical Therapist (PT) providing therapy services to clients in a home care setting.
Responsibilities:
Deliver client-centered therapy to restore or improve independence in daily activities within scope of practice, utilizing systems like Sansio EMR and Microsoft applications. Participate as part of a dynamic team to enhance patient outcomes.
Qualifications & Skills:
Graduate of an accredited Physical Therapy program, current licensure in Colorado, previous home care experience, valid driver’s license, and auto insurance. Proficiency with Microsoft tools and EMR systems is required.
Key Duties:
Provide therapy services, document patient progress, adhere to dress code standards, and utilize provided equipment such as iPads. Maintain professional standards and contribute to patient recovery goals.
