Job description
Proforma Health Inc. seeks a Home Health Physical Therapist to deliver patient-centered therapy in clients' homes in Colorado. You will document progress using Sansio EMR and Microsoft tools, and work as part of a collaborative team to improve patient outcomes.
The role requires a PT degree, Colorado licensure, and valid driver’s license with auto insurance; home care experience is preferred.
