Job description
Role: Travel Home Health Physical Therapist responsible for patient recovery, managing chronic conditions, and overseeing care in diverse settings.
Responsibilities: Conduct resumption of care, start of care, discharges, and supervise PTA, ensuring quality patient outcomes.
Work Schedule: Monday through Friday, with no weekend or on-call duties.
Coverage Area: Marion and Hamilton counties.
Productivity: Meet 30-point productivity expectations.
Travel & Reimbursement: Mileage reimbursed between visits; travel and license expenses covered.
Qualifications & Skills
Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited institution.
Active State License and BLS certification (may be required).
Home health experience and proficiency with EMAR system (HCHB).
Key Attributes
Compassionate, detail-oriented, and capable of working independently in a mobile environment. Ability to manage patient care, documentation, and coordinate with healthcare teams.
