Responsibilities
Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, aligning with department objectives
Serve as a co-leader in an interdisciplinary approach to patient groups, coordinating care with other team members
Assess patient needs and develop/adjust treatment plans tailored to the home health context and inpatient rehab components
Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence
Document patient progress, outcomes, and plan of care; communicate findings with patients, families, and the care team
Participate in discharge planning and patient/caregiver education to support continued recovery after discharge
