Job description
Role
Travel Physical Therapist specializing in Home Health services, serving a rural region across four counties.
Responsibilities
Providing patient care in home settings, conducting Oasis Assessments, and managing Start of Care cases.
Qualifications
Valid physical therapy license
BLS (AHA) certification
Minimum of 1 year home health experience
Skills
Experience with Homecare Homebase (HCHB) charting system preferred
Proficient in using tablet technology for tracking visits
Capable of managing an average of 4-6 patient visits daily
Key Duties
Serve elderly patients with focus on falls and fractures
Perform assessments
Deliver care during daytime hours, Monday through Friday.
Additional Info
Business casual or Hunter green scrubs, with flexibility for shift cancellations.
