Job description
Role: Travel Physical Therapist responsible for providing home health services in a flexible schedule, focusing on conditions such as congestive heart failure, orthopedic issues, and pneumonia.
Responsibilities: Conduct patient visits including start of care, recertifications, evaluations, and daily visits, typically 6.5 per day/week. No clinical involvement outside patient visits. Use Homecare Homebase charting system. Participate in a two-week orientation with a preceptor and available support from clinical managers or educators 24/7.
Qualifications: Valid license, BLS certification, home care experience preferred. Must have home care experience and home base knowledge.
Skills: Strong patient assessment, documentation, and time management skills. Ability to work independently within a home health setting.
Key duties: Deliver patient care in a home environment, manage schedules including rotating weekends and holidays, and ensure compliance with clinical protocols.
