Job description
Role: Travel Physical Therapist specializing in Home Health services, providing patient treatments in home, assisted living, and care facilities.
Responsibilities: Delivering evaluations, treatments, and developing care plans for approximately 3-5 patients daily within a designated territory. Supervising up to two therapy assistants, coordinating visits such as Start of Care and assessments, and working as part of a dynamic care team. Travel within assigned areas is required, with mileage reimbursed. Orientation includes online training and field precepting, with independent work expected after initial phases.
Qualifications & Skills: Valid California PT license, BLS certification, NPI, driver’s license, and automobile insurance. At least 2 years of experience in healthcare, including home health and OASIS knowledge. Strong communication, organizational skills, and ability to work in varied home environments. Experience with Electronic Homecare Home Base (HCHB) is preferred.
Key Duties: Conduct patient visits, assessments, and treatment plans, ensuring quality care in a flexible schedule that may include one weekend per month and holiday work with time-off flexibility.
