Description
American Traveler is seeking a Physical Therapist for Home Health with HCHB and OASIS experience to serve a geriatric patient population.
Details
Home health setting covering the Mossyrock & Morton Area
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
Caseload of approximately 5-7 patients per day
Patient population is geriatric
On-call required every 6th weekend
Uses HCHB (Homecare Homebase) EMR system
Traveler expected to be flexible with territory assignments
Requirements
BLS certification required
Minimum 1 year of recent relevant experience required
HCHB (Homecare Homebase) proficiency required
OASIS experience required for completing Start of Care assessments
Active driver's license and personal transportation required
License required if applicable
Timely documentation and communication required
Additional Information
Responsibilities include home exercise programs and patient/family education
Treats conditions including orthopedic, neurologic, and post-surgical cases
Monday orientation required, including completion of billable modules
Hospice/Home Health checklist completion is mandatory
Scrubs required in any color, no jeans permitted
iPad provided;
cell phone not provided
Mileage reimbursed at the IRS standard rate
First-time travelers will not be considered for this role
Local candidates will not be considered
Travel pairs accepted, but each traveler must have their own territory and transportation
Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
Requested time off must be submitted at the time of application
