Job description
American Traveler is seeking a Home Health Physical Therapist with 2+ years of home health experience for a 13-week travel assignment.
Requirements
Minimum 2 years of PT experience, with prior home health experience required
Current BLS certification required
CA PT license required if applicable
Valid driver's license and car insurance required
Active state or compact license required at time of submission; pending licenses not accepted
Responsibilities
Home Health care setting serving patients of all ages with multiple comorbidities
Caseload of 4-5 patients per 8-hour day
Documentation must be completed within 1 day of patient visit
Uses HCHB (HomeCare HomeBase) EMR
Schedule is Monday-Friday, 8am-5pm, 5x8-hour day shifts
Support from PTAs/COTAs and 24/7 manager availability
Mileage reimbursed per GSA rate
Responsibilities include standard physical therapy treatment and timely clinical documentation
All MyJourney training modules must be completed prior to arrival; travelers may be delayed if incomplete
Copy of driver's license required for consideration
Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and rehire eligibility
Facility may cancel up to 4 shifts per 13-week assignment
