Travel Physical Therapist – Home Health
Willits, CA
We’re seeking an experienced Travel Physical Therapist for a Home Health assignment in Willits, California. This is a great opportunity for a PT who enjoys autonomy, patient-centered care, and working in a community-based setting.

Location: Willits, CA
Setting: Home Health
Schedule: 8x5s (8-hour shifts, 5 days per week)
Duration: 13 Weeks
Requirements
Home Health experience – Required
Active California PT License – Required
BLS/CPR Certification – Required
Valid Driver’s License – Required
Must be 21 years of age or older
Essential Functions
Master’s Degree – Preferred
Complete comprehensive patient evaluations
Develop and implement individualized treatment plans
Address assessment areas requiring clinical attention
Maintain accurate and timely documentation
Ensure compliance with state, federal, and agency guidelines
Collaborate with interdisciplinary teams to support patient outcomes
