Job description
Fairview Hospital / Cleveland Clinic seeks a Home Health Physical Therapist to provide skilled therapy to patients in home settings across Colorado. The role requires evaluating patients, creating and implementing personalized treatment plans, and delivering care to enhance mobility and function.
Applicants must hold a Colorado license, have at least 2 years of experience, BLS certification, and familiarity with the Meditech EMR system. Local candidates are not considered.
