Job description
Apex Home Health, Hospice & Home Care is seeking a Travel Physical Therapist to provide home-based therapy for adults with chronic conditions. You will conduct patient visits (about six daily), perform OASIS-E assessments, and document in Homecare Homebase using Samsung Note 9 devices.
Responsibilities include travel within York County and surrounding areas up to 100 miles as needed, with a day shift of 8 hours and minimum 40 hours per week. Join a dedicated home health team.
