Physical Therapist – Outpatient, travel assignment in a hospital setting in Ukiah, California, United States. Responsibilities
Plan and conduct segments of outpatient physical therapy treatment programs to restore and improve physical, social, and mental functions while aligning with department objectives.
Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with team members to optimize recovery.
Assess patient status, establish treatment goals, implement plans of care, and monitor progress toward functional outcomes.
Document evaluations, treatments, progress notes, and outcomes in the medical record with accuracy and timeliness.
Educate patients and families on home exercise programs, safety considerations, and activity modifications.
Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care and adjust plans as needed.
Requirements
Active California physical therapy license or eligible for licensure (as applicable)
Doctor of Physical Therapy (DPT) or equivalent degree
Outpatient physical therapy experience preferred
Ability to work 40 hours per week in a day shift
Comfortable collaborating within an interdisciplinary team
Ability to work in a hospital-based outpatient setting
