Join a 13-week travel Physical Therapist (PT) assignment in Tomahawk, WI, with 8-hour day shifts in a skilled nursing facility (SNF). Tomahawk is known for its beautiful lakes, outdoor recreation, and friendly community atmosphere. As a PT in a skilled nursing facility, you will develop individualized treatment plans to help residents restore physical function after injury or illness.
You will work closely with physicians and other healthcare professionals to assess mobility, pain levels, and range of motion, then use exercises, stretches, and advanced rehab techniques to improve strength, flexibility, and coordination. Your role includes focusing on fall prevention, pain management, and helping residents regain independence for daily activities.
You may also recommend home safety modifications and collaborate with families to support safe discharge planning. PTs treat both short- and long-term residents, adapting plans to each person's progress and needs, and document treatment outcomes to ensure quality care and regulatory compliance. Qualifications typically include a current PT license and strong skills in patient assessment, treatment planning, and communication.
Experience with therapeutic exercises, adaptive techniques, and collaboration with other healthcare professionals is recommended.
