Physical Therapist position in a Skilled Nursing Facility in Lebanon, New Hampshire, United States. This travel assignment starts 10/05/2026 and runs for 14 weeks, with a weekly pay range of $2,633 to $2,788 and guaranteed hours of 35 per week. Responsibilities
Conduct comprehensive physical therapy evaluations for residents and develop individualized treatment plans targeting mobility, strength, balance, and functional independence.
Implement and adjust therapeutic interventions including therapeutic exercises, gait training, manual techniques, and transfer training as appropriate.
Monitor and document patient progress, update treatment plans, and contribute to discharge planning in collaboration with the interdisciplinary team.
Perform timely, accurate documentation of assessments, goals, progress notes, and outcomes in accordance with facility policies and regulatory standards.
Collaborate with physicians, nurses, therapists, and other team members; participate in care conferences and family communications as needed.
Ensure safety and proper use of equipment, maintain infection control practices, and adhere to all facility policies and ethical standards.
Requirements
Active physical therapy license in the United States per state requirements.
Degree in physical therapy from an accredited program (DPT or MPT).
Ability to work in a skilled nursing facility setting with older adult residents.
Strong documentation, communication, and teamwork skills.
Flexible and able to participate in a travel assignment for a 14-week contract with the specified schedule.
