Overview
Physical Therapist for a Skilled Nursing Facility travel assignment in Unity, New Hampshire, United States. Start date 10/05/2026 for 14 weeks. Weekly pay range $2,594 – $2,715. Guaranteed hours 40 per week. Shift is 5x8 Days with a schedule from 08:00 to 16:30. This is a travel - temporary assignment.

Responsibilities
Perform comprehensive physical therapy evaluations and re-evaluations for residents in the skilled nursing setting.
Develop, implement, and modify individualized treatment plans focusing on functional mobility, gait, balance, strength, and activities of daily living.
Apply evidence-based therapeutic interventions and modalities within scope of practice; document progress accurately and timely.
Monitor patient response, adjust treatment plans as needed, and communicate with the interdisciplinary team including physicians, nursing, and therapy staff.
Ensure patient safety, establish goals, and participate in care planning meetings; maintain compliance with regulatory and facility policies.
Requirements
Active physical therapy license in New Hampshire (or eligibility for licensure)
Doctor of Physical Therapy (DPT) degree or equivalent
Minimum 1 year of physical therapy experience in a clinical setting (preferred)
Experience in skilled nursing facility or geriatric rehabilitation preferred
Strong communication and documentation skills; ability to work both independently and as part of an interprofessional team
