Description
American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of experience and a valid Ohio state license.
Details
Full-time Home Health PT position covering select Cincinnati, OH zip codes (45202, 45210, 45214, 45217, 45219, 45220, 45229, 45206, 45207, 45212, 45237)
Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
HomeCare HomeBase charting system experience preferred but not required
Requirements
Minimum 1 year of experience required
BLS certification required
Valid state license required prior to start date;
cannot start with a pending license
License required if applicable
Experience with home health and acute care settings preferred
Skilled in documentation including assessment, discharge, evaluation, long and short-term goals, OASIS, and plan of care
Proficient in use of assistive devices including crutches, gait belt, rolling walker, standard walker, and wheelchair assessment
Knowledgeable in home exercise programs, patient and family education, and safety and injury prevention
Familiarity with Medicaid, Medicare A, and Medicare B billing
Additional Information
Responsibilities include conducting home-based assessments, developing treatment plans, and completing OASIS documentation
Focus on patient and family education, safety planning, and injury prevention
