The Position
Case Management Nurses coordinate long‑term care for their patients. Their goal is to treat patients at optimal times to keep them healthy and out of the hospital. As a Case Management Nurse, you’ll coordinate care for specific patient groups, work with specialties such as HIV/AIDS, cancer, geriatrics or pediatrics, research the latest treatments, and partner with insurance companies to secure the best, most cost‑effective care possible.
Responsibilities
Coordinate comprehensive, long‑term care plans for assigned patients.
Collaborate with specialists, hospitals, and community resources to ensure seamless transitions.
Review treatment protocols and stay current on evidence‑based practices.
Advocate for patients’ medical and insurance needs to secure optimal care.
Qualifications
Minimum of 1 year of current work experience in CASE MANAGEMENT.
New Hampshire State Healthcare Provider license or willingness to obtain one.
Certifications Needed
BLS
