Hourly Pay Rate: $67
*Rate includes $202 daily tax-exempt stipend
Requirements:
Active RN license in the State of New York
UR/CDI Experience, CDI Certification Preferred
Responsibilities:
Perform comprehensive assessments of patients' medical, psychosocial and discharge needs.
Advocate for patient needs and appropriate resource utilization.
Develop, implement and monitor individualized care plans to facilitate safe, timely and effective patient transitions across care settings.
