Description
American Traveler is hiring an RN Care Coordinator for a Case Management/Utilization Review role requiring at least one year of recent acute care case management experience and a current CA RN license.
Details
Acute care hospital setting within the Case Management department, with potential assignment to the Emergency Department
Coordinates care and services for assigned patients throughout an episode of illness or treatment
Caseload includes quality/utilization review, admission/discharge criteria, observation/1-day stays, and 30-day readmissions
8-hour shifts scheduled 8am - 4:30pm, including weekdays and weekends
Uses Epic EMR
Requirements
Active CA RN license required
Current CPR certification required
At least one year of recent Case Management experience in an acute care setting required
Additional Information
Collaborates with patients, families, physicians, payors, and the multidisciplinary care team to coordinate services
Advocates for patients and families throughout the episode of care until discharge or transfer to an appropriate level of care
Initiates clinical pathways based on admission diagnosis and serves as a resource for utilization management
No holiday coverage required
Candidates must be W-2 employees of their staffing agency
