Start Date: September 21, 2026
Location: Chico, California
Schedule: 8-hour shifts, 8:00 AM - 4:30 PM, weekends and weekdays. 40 guaranteed hours per week. 13-week assignment (91 days).
Highlights:
Coordinate care and services for assigned patients throughout their episode of illness or treatment
Collaborate with patients, families, physicians, care teams, and payors to achieve optimal clinical outcomes
Utilize advanced clinical skills and nursing process principles for assessment, planning, intervention, and evaluation
Evaluate quality and utilization cases, including admission/discharge criteria, readmissions, and Medicare compliance
May include Emergency Department assignment with case management responsibilities
Serve as patient advocate and resource throughout the entire care continuum
Requirements:
Current California RN license
Current CPR certification
Minimum one year of recent Case Management experience in an acute care setting
Must be W-2 employee of staffing agency
