Job description
American Traveler is hiring an RN Case Manager for an acute care hospital, requiring a NJ or compact RN license and prior inpatient discharge planning experience.
Requirements
Active NJ or compact RN license required
BSN required
CCMC certification preferred
Minimum 2 years of acute inpatient hospital discharge planning experience required
Hospital experience required, including discharge planning and placement in rehabilitation facilities
Experience with subacute patients required
Case Management and Managed Care experience required
In-Patient Case Management experience required
Responsibilities
Hospital-based Care Management unit
Coordinates patients through acute care hospitalization
Manages all aspects of discharge planning and transition process
Performs utilization management and quality screenings
Works with subacute patients and discharge placement to rehabilitation facilities
AM shift, 8:00 AM to 4:00 PM, 37.5 hours per week
Rotating weekends required every 4th or 5th weekend
One summer and one winter holiday coverage requiredMay float to a sister hospital as needed
No on-call requirements
Uses EPIC for charting
Coordinates comprehensive discharge planning and transition of care for acute hospital patients
Performs utilization review and quality screening as part of daily responsibilities
Local candidates accepted
Travelers may extend but are limited to one calendar year on assignment before requiring a break
